Indikation & Zulassung

Neoadjuvante Chemotherapie (S3-Leitlinie Statement 9.7, Level of Evidence 1++) Dosisdichtes MVAC (ddMVAC) ist eine etablierte neoadjuvante Option beim muskelinvasiven Harnblasenkarzinom (MIBC) bei Cisplatin-geeigneten Patienten. Geringere Toxizität als konventionelles MVAC bei vergleichbarer Wirksamkeit. Obligate G-CSF-Prophylaxe.

🚨 Wichtige Warnhinweise
  • Kumulative Doxorubicin-Dosis: Max. 450–550 mg/m² (Kardiotoxizität). Echokardiographie vor Therapiebeginn.
  • Febrile Neutropenie: Notfall – sofortige Antibiotikatherapie (Piperacillin/Tazobactam oder Cefepim)
  • Methotrexat-Toxizität: Bei Nierenfunktionsstörung stark erhöht – CrCl vor jeder Gabe prüfen
